Vital Signs
Vigil allows staff to document vital signs alongside observations, with automatic detection of abnormal values.
Recording Vitals
- Open patient details
- Tap Vitals tab
- Enter available measurements
- Tap Save
All fields are optional—record what you measure.
Vital Sign Fields

Abnormal Value Detection
When you enter a value outside normal ranges, Vigil:
- Highlights the field in red
- Displays the normal range
- Asks for confirmation before saving
- Creates a vital critical alert after saving
This ensures staff consciously acknowledge abnormal values and that appropriate follow-up is triggered.
Critical Thresholds
Values outside these ranges trigger critical alerts:

Pain Assessment
The pain scale (0-10) follows standard clinical practice:

| 4-6 | Moderate pain |
| 7-10 | Severe pain |
Pain scores do not trigger automatic alerts but are visible in patient records and handoff reports.

View a patient's vital sign trends:
- Open patient details
- Tap Vitals tab
- Scroll to see recent readings
- Abnormal values are highlighted
EMR Sync
When EMR integration is enabled, vitals automatically sync to the EMR:
- Vitals appear in EMR flowsheets
- Sync happens within minutes of documentation
- Failed syncs retry automatically
- Sync status visible in Integration settings
Notes with Vitals
Add context to vital readings:
- Patient positioning during measurement
- Equipment used
- Patient complaints or observations
- Factors affecting readings (activity, anxiety, etc.)
