Clinical Assessments
Vigil includes standardized clinical assessments with automatic scoring.
Available Assessments

Starting an Assessment
- Open patient details
- Tap Assessments tab
- Select an assessment type
- Complete questions one at a time
- Review results and save
Assessment Flow
Each assessment presents one question per screen:
- Read the question/item
- Select the appropriate response
- Tap Next to proceed
- Progress bar shows completion status
Scoring
Vigil automatically calculates:
- Total score based on response values
- Severity level based on score thresholds
- Clinical interpretation for context

- Negative screen
- Low risk
- Moderate risk
- High risk
CIWA-Ar (Alcohol Withdrawal)

Results
After completing an assessment, you see:
- Total score
- Severity level with color coding
- Clinical interpretation
- Option to add notes
Tap Save Assessment to record.
Assessment Scheduling
Assessments can be scheduled to repeat:
- Hourly - For acute monitoring (CIWA, COWS)
- Every shift - For ongoing risk assessment
- Daily - For routine screening
- Weekly - For periodic evaluation
Scheduled assessments appear as due items in the patient's assessment tab.
Assessment History
View completed assessments:
- Open patient details
- Tap Assessments tab
- See completed assessments with:
- Date and time
- Score and severity
- Staff who completed
Tap an assessment to view full responses.
EMR Sync
Assessment scores sync to the EMR as discrete data:
- Score appears in appropriate EMR location
- Severity level included
- Individual responses available for review
